Claude Code changed how Auto Mode decides which actions require approval or further handling.
In version 2.1.278, Anthropic makes server-side classification the default for API and Enterprise users, as well as Bedrock, Vertex, Microsoft Foundry and gateways environments.
According to Anthropic, server-side classification is not itself billed, although the system may still fall back to a billable alternative in certain situations.
The /status command now also shows where classification takes place.
